Yes. The consultation is $150, and it is credited in full toward any treatment performed the same day. It buys you a private, one-on-one assessment and a personalized plan from the physician — not a sales pitch.
Who will I see at my appointment?
Dr. Caio Trentin, MD. He performs every consultation and every treatment at FORMA personally — never a delegated injector. You meet the physician at your first visit and at each one after.
Can I be treated the same day as my consultation?
Often, yes. Many treatments can be performed during the same visit once Dr. Trentin confirms you are a good candidate. Some plans are staged over multiple sessions; he will tell you which applies to you.
